Sports Physical

The sports phyisical form must be completed every two years in order for the student to be eligible to participate in athletics.


Medical Release Form

The medical release form needs to be signed and returned to participate in MRLH sports.


Concussion Release Form after watching the VIDEO

The concussion release form needs to be signed by participants and players and returned to participate in MRLH sports.


 

Fall/Winter Sports Program Form

The Fall and Winter Sports Programs at MRLH are a Booster fundraiser that provides a great opportunity to advertise.


Booster's Volunteer Survey Form

 

The Hawk Booster Club is a group of parents, alumni, and friends who support Mount Rainier Lutheran High.


 

Hawk Club 

The Hawk Club is a Booster fundraiser that takes advantage of the fact that many families plan to pay admission to most home games during volleyball and basketball seasons.


 Hawk Apparel 

Get your Hawk apparel to show your school spirit.


Dance Form and Visitor for Dances Release

This form must be completed and turned in to the supervising teacher at least 2 days prior to the dance. This form requires the signature of the visiting student, parents, and the visiting student's principal.

Help support the Hawk Yearbook and advertise your business or submit a personal ad.


Yearbook Sponsorship Form

Our goal at MRLH is to make the yearbook affordable for each student on our campus.  We would like you to consider sponsoring the yearbook to help defray the costs of publishing.
